It enables the lens to reach 1:1 magnification (for example a fly will appear life size on the image frame), while without it, the lens can only reach 1:2 magnification (the fly will be half life size). If you need such magnification, you put the adapter on the lens and then mount the whole stack on the camera. Notice the lens sits "upside down" when mounted on the adapter. Without the adapter the largest aperture is f3.5, with the adapter it is f5.6. This difference corresponds to light falloff that happens with the adapter on.
