Job Specification Job Title: Disease Management Case Manager – Cardiovascular Department: Clinical Risk Management Manager: Clinical Risk Manager Education, Qualifications and Experience: Code 8 Driver’s License Matriculation Registration with the South African Nursing Council Registered Nursing Sister with disease management experience and cardiovascular qualification 2-3 years medical aid/ managed healthcare experience essential Knowledge and Skills: Excellent command of the English language Fluency in another language would be an advantage Excellent communication skills Computer literacy essential Logical thinker Good time management skills Good organisational skills Excellent report writing skills Knowledge of Cardiovascular medication, treatment plans and protocols Knowledge of Case management procedures Knowledge of Prescribed Minimum benefit protocols and algorithms would be an advantage Personality Requirements: Portray a professional image Good interpersonal skills Able to handle stress and pressure Able to apply clinical knowledge Ability to work independently, efficiently and consistently Ability to develop good working relationships with work colleagues – a team player Able to maintain a high level of confidentiality and responsibility Honest/compassionate/empathetic Pro-active Able to problem solve Logical thinker – level headed, mature and patient Ability to make objective decisions A self motivated person who takes personal responsibility for developing excellence Demonstrates an interest in continuing professional education Duties: Constant & ongoing development of the Disease Management Programmes Communicating with prescribers, patients and the Fund with regard to the programme rules; disease education and related clinical information Production of educational material and dissemination to patients Retrospective review of related claims Developing and maintaining clinical protocols including new drug reviews Working with other healthcare personnel within the organisation Liaising with pharmaceutical companies Compilation of monthly reports detailing programme progress and areas of cost containment Be able to manage all related hospital admissions To keep abreast of benefit changes To develop and deliver educational presentations to employer groups.
