CO ORDINATOR PROFESSIONAL NURSE X2 IN SOUTH AFRICA

OVERVIEW: A Coordinator is a community based outreach worker who is responsible for a group of between thirty – forty five Mentor Mothers (MM) and their associated Assistant Coordinators. Their aim is to manage and support their Assistant Coordinators and MMs ensuring that they are held accountable for the tasks set out in their Job Descriptions Fieldwork · Visit (mothers-to-be) M2B, malnourished children and home based care cases that seem to be problem cases needing extra support. Assess needs of client and ensure that intervention is done correctly, add extra intervention and medical advice as necessary or refer to a clinic or hospital. Attend to high risk clients as an absolute priority · Visit with an assistant coordinator one day of the week and capable MMs for the other 3 days of the week. Do 8 – 10 visits per day · Ensure community discussions are held as and when the opportunity arises. · Provide assistance on complicated social cases referred by Assistant Coordinators and MMs · Mediate any conflict within the programme · Take responsibility for the support and supervision of between 30 – 45 MMs and their associated Assistant Coordinators. · Ensure all MMs are doing six follow up visits a day three days a week and 10 house to house visits a day two days a week · Hold all Assistant Coordinators accountable for their full job description · Have bi-weekly supervision sessions with Assistant Coordinators and assess them quarterly · Report poor performance of assistant coordinators and mentor mothers immediately to the Programme Manager/ Administrator and arrange for disciplinary action where necessary · Follow up on poor performance- initiate disciplinary reviews with HR · Conduct spot checks to ensure Mentor Mothers and Assistant Coordinators are performing the tasks required of them. · Meet once a month with each of your coordinators and their MMs to discuss specific problems and do continuous education · Collect monthly visiting plans from each assistant coordinator before the first of every month ensuring that every MM is visited twice a month · Collect folder reporting forms and daily visiting forms every afternoon and check them. The reports must describe the visits and have a performance assessment of the MM · Ensure that Assistant Coordinators are completing folder reporting forms on a daily basis · Ensure that Assistant Coordinators are in the field doing a 6-10 visits per day, do spot checks · Ensure old cases are taken over by a new MM and are handed over properly · Follow up on ‘locked doors” and “gone to Eastern Capes” to ensure that this is the correct situation · Ensure the employees that you are managing understand what they are doing and are doing · Ensure that Assistant Coordinators know their MMs and are managing and checking up on them · Assist in the closure of folders and management of caseloads Qualification; · A relevant tertiary level qualification (Professional Nurse) Computer literate · English & Xhosa written & spoken · Excellent Communicator · Organised, Good time management skills, ability to plan · Active member in her community · People Management & strong interpersonal skills · Teamwork & Leadership Send CV to: johannes@poundrecruitment.co.za
